If you do not have access to a veterinarian, there are visual signs you can look for.
There is a body condition scoring method (above) that can help you see specific areas.

You should not be able to easily see the ribs, the pin bones, the tail head
Equines should walk in an upright position on all 4 legs, free of discomfort
Hooves should be short enough to allow adequate movement without discomfort
What are the reasons for an unhealthy horse/equine ?
Age can be a consideration; Physical attributes change over time making digestion difficult for horses
Teeth can be remedied and feed can be altered to sustain a horse. See your veterinarian
Lack of adequate forage, clean water, farrier, dental care or regular prescribed treatment of parasites.
Lack of adequate shelter in severe weather conditions may be a cause
